Arrive Early: Please arrive 5-10 minutes before your first consultation to fill out a new patient form, or download the PDF version below and complete this and email this to reception@wahealthgroup.com.au or bring a hard copy with you to your appointment.

Notify Reception: When you arrive report to the receptionist at the front desk.

Referral Documentation: Please bring all applicable documentation in hard copy with you to your appointment on the day. WA Health Group is a paperless practice, and copies of these documents will be electronically scanned to your patient file.

Parking - Canning Vale: WA Health Group Canning Vale is located at Suite 7, Level 1, 2 Queensgate Drive in Canning Vale. Entering into the Queensgate Centre, take the ramp up to level one for ease and readily available parking during business hours. Disabled parking bays are only available on the ground level, please take the lift to level 1.

Exercise Physiology Patients: Please come dressed in attire that you are comfortable moving in, and wear enclosed footwear for gymnasium entry.
