Do you have Diabetes and want to get more active?

You may be eligible for Medicare funding

Patients with Type 2 Diabetes and a management plan from their general practitioner (GPMP) may be referred for group allied health services. This entitles you to an assessment service, and up to eight (8) group services per calendar year covered by Medicare. The diabetes class incorporates a combination of strength and aerobic exercises to help manage your diabetes.

How can exercise help my Diabetes?

Regular exercise improves efficiency of insulin which can lower blood glucose and improve HbA1C as well as helping with the prevention and management of other conditions associated with an inactive lifestyle.

How much does it cost?

With a GP referral, Medicare rebate covers the bulk of appointment costs. There is a small gap additional charge per appointment on top of Medicare rebate.

How do you join? Simply print the attached form and take it to your GP.
